This information is required by the Fort Monmouth Directorate of Public Safety and will be used for security purposes only. All passengers 16 years and older must present a government or school-issued photo ID prior to being granted access to the installation. All vehicle passengers must be listed on the form. Drivers must have driver's license, vehicle registration, and proof of insurance before being granted access to post.

If you are driving a rental car to Fort Monmouth, please annotate this on the form and ensure you have the vehicle registration and insurance information from the rental company. If possible, please list the rental company from where you are renting.
