DA Form 1559 - Inspector General Action Request

The DA Form 1559 is used to secure sufficient information to make inquiries into matter presented to the IG and to provide a response to the requester(s) and/or take action to correct deficiencies. A person who make a request for IG assistance is the requester. When the requester presents the IG with his or her allegation(s) or issue(s) the IG will ask the requester to fill out a DA Form 1559. If the requester contacts the IG telephonically, the DA Form 1559 is filled out by the IG taking the call. The information provided on this form provides the IG his or hers initial base of knowledge to begin an inquiry.

You may fill out a DA Form 1559 prior to your visit to the IG or for mailing or faxing to the IG. If you do this, please ensure all information is filled out completely and use this link DA Form 1559 . See the instructions below on how to properly fill out this form. The form to the left is a DA Form 1559-R. Each field in the form is identified with a letter. Match the letters in the fields with those below for information on the data which should be entered.

Enter your current rank. (Unless you wish to remain anonymous.*)
Enter your SSN. (Be sure and read the Privacy Act Information above.) (Unless you wish to remain anonymous.*)
Enter your Component/Status
Enter your complete military or civilian mailing address. (Unless you wish to remain anonymous.*)
Enter your duty phone number or any other appropriate contact numbers. (Unless you wish to remain anonymous.*)
Enter your preferred mailing address. (Does not have to be your unit address)
Enter an e-mail address where we can contact you if preferred.
Ask yourself, "What do I want the IG to do for me?", and put this in very concise language here.
Collect your notes and thoughts and write all relevant information to your issue in this location. Feel free to use continuation sheets if necessary. Remember to include your current status, other agencies you have contacted, if you contacted your chain of command, and if you given your chain of command an opportunity to address the problem. Finally, be sure to have supporting documentation relevant to your situation.
